  Free Vaccines for Life
A Westfield Veterinary Hospital Patient Wellness Program 		 
       
  How it Works
* You bring your pet in for a physical examination
* You enroll your pet in the Free Vaccines for Life program by paying the $49 enrollment fee
* You agree to bring your pet back to Westfield Veterinary Hospital at least once every 12 months for an examination
* Your pet receives free vaccinations for the rest of its life 		 
       
  Included Vaccinations  
  Dogs:
Annual or 3 year Rabies
Annual Bordetella (Kennel Cough)
Annual Leptospirosis
Annual or 3 year DAPP
(distemper/parvo/hepatitis/parainfluenza)
All Puppy Boosters of Above Vaccines		 Cats:
Annual or 3 year Rabies
Annual Leukemia
Annual or 3 year FVRCP
(distemper/rhinotracheitis/calicivirus)
All Kitten Boosters of Above Vaccines
  
       
  What Else Should I Know?
*Lyme vaccines, or any special order vaccines, are not included
*All examinations, diagnostic tests, services, and products other than vaccinations will be charged according to WVH's regular fee schedule
*Enrollment in Free Vaccines for Life is not transferable or exchangeable between pets or owners.
*Free Vaccines for Life is only available at WVH, and cannot be used at any other hospital or clinic.
* Your pet must come to WVH for an examination at least once every 12 months to maintain membership.
  
       
  What happens if my pet doesn't make it in to see the doctor before a year passes?
There is a thirty day grace period before your pet's membership expires. If your pet does not come in to be examined by the doctor within a month of it's due date, then your pet's membership will expire. You would then have to pay regular prices for all vaccines from that point on, or if the program is still available you could re-enroll your pet.		  
       
  Do I have to pay again every year? What else do I have to buy?
There is no annual or recurring membership fee. Once you have paid the $49 enrollment fee, your only committment is a single physical examination annually. You would be responsible for the examination fee.		  
       
  If I enroll my pet today, do I have to wait until next time to get free vaccines?
No. You can take advantage of the free vaccines immediately.		  
       
  How long will this program be available?
Once enrolled, your pet is enrolled for life. WVH reserves the right to stop accepting new enrollment at any time		  
       
  Do I have to wait until my pet is due for vaccinations before I can enroll him?
No. If your pet has been examined by WVH in the last 12 months, you can simply stop in and pay the enrollment fee. (Your pet's enrollment expiration date will be 12 months from the time of its last examination). Otherwise, you only need to call and schedule your pet's examination and you can enroll your pet at that appointment.
